Your Information: Include your name and the account associated with the deletion.
Patient Summary URL: Provide the URL of the patient summary page for the affected patient. You can find this page by using the search feature to locate the patient.
Estimated Deletion Date: Let us know roughly when the case was deleted.
Important:
We can typically restore deleted cases if we can identify the deleted appointment ID and if the deletion occurred within the last 30 days.
