Adding A Provider Signature To A Patient Consent Form

If you are a provider who likes to have your signature along with the patient's on specific consent forms this article will show you how.

There are 3 options within Xchart that can be utilized to input the providers signature with the patient's signature on consent forms. You can choose which option works best for you and your team.

🌟OPTION 1: Having 2 of the same consent forms in the patients chart, one signed by the patient and one signed by the provider.

  1. Login to your dashboard at Xchart.com

  2. Select the patient you would like to have sign the consent form

  3. In the Case Summary page "send" send the consent form to the patient you would like to have signed.

  4. Once the consent form you've sent has been received and signed by the patient the colored box under the consent forms name will turn green and say complete.

  5. Next (still on the case summary page) select "add consent form" and add the exact consent form you had the patient sign. NOTE: This step and the following steps may happen at a separate time depending on how long it takes the patient to sign their consent form.

  6. You should now have 2 of the same document on the case summary page. One that has already been signed and completed by the patient and one that has just been created.

  7. Instead of sending the second consent form to a patient, once the anesthesia provider has received and reviewed the original consent form from the patient they can "open" the additional consent form from any device they are on and sign themselves with their name.

  8. Put their name and title in the freeform text box

9. Once signed this is what it will look like with their signature, then select "submit"

10. Now you will see two of the same consent forms say "complete" on your "case summary" page.

11. All documents can be downloaded as one PDF file.

🌟OPTION 2: Having both the patient and the provider sign the consent at the same time in the same signature box.

NOTE: make sure the patient and the anesthesia provider are together to be able to sign at the same time. This can be done on any device however it can be easier if done on a touch screen iPad or tablet to hand to the patient and anesthesia provider.

  1. Login to your dashboard at Xchart.com

  2. Select the patient you would like to have sign the consent forms

  3. In the patients Case Summary page make sure the consent form has been added to the patient's chart. (If it needs to be added still you can simply add it by selecting "add consent form" and find the one you need). Then select "open" on the particular consent form.

  4. On the consent form for the "signer name" input the patients name AND the anesthesia providers name and title.

  5. Select "add your signature." The provider can sign at the top of the box, while the patient signs along the line in the box. HINT: have the provider sign first to eliminate the patient saving the signature prior to the provider signing it.

  6. Select "submit"- now there will be 2 signatures attached to this one patient consent form when you download the file.

🌟OPTION 3: Create your own consent form under your organization that all providers can utilize and sign saying they have received and reviewed the consent forms the patient has signed prior to anesthesia. NOTE: We encourage you to run this by your legal team to make sure this is legal in your state as a way to add the providers signature for consent forms.

  1. Login to your dashboard at Xchart.com

  2. Select "Patient Forms" from the blue bar on the left. Reminder: only members who are admins can add patient forms to the entire organization.

  3. Follow the prompts on the page to create your custom form for anesthesia providers to sign once they have received and reviewed patient consent forms.

  4. Once this has been created and saved for the organization it can then be added to a patients chart for the provider to sign.

  5. From the dashboard select the patient that you will be providing sedation for.

  6. Once in the "Case Summary" page select "add consent form"

  7. Find the custom consent form that was created and add it to the patients chart.

  8. Once added select "open" to have the anesthesia provider sign the form.

  9. Put the anesthesia providers name and title

  10. Add signature

  11. Click "submit"

  12. Now you have a provider signed document that can be downloaded with all the patient's consent forms and final sedation record once complete.
