A More Patient-Centered Approach

Xchart was originally designed around anesthesia records, with health history data gathered solely to support those records. However, over time, it became clear that storing health history directly within anesthesia records was not ideal. Instead, this information should be linked to the patient, ensuring it remains accessible for future appointments—even if anesthesia records are moved, canceled, or deleted.

Transitioning this core part of the experience without disruption has been a challenge, but we're excited to roll out this improvement.

Key Benefits

  • Minimal Change to Your Workflow – If you prefer the old process, you can continue accessing health history from the anesthesia record as before. It will look slightly different, but everything remains familiar.

  • Health History Independent of Appointments – You can now complete a patient’s health history directly from the Case Manager, without opening an anesthesia record. This ensures the history is always linked to the patient, regardless of how many appointments they have.

  • Versioned & Immutable Records – Once a health history is signed, it cannot be modified. Instead, updates create new versions, allowing you to track changes over time.

  • Simplified Review Process – When completing an anesthesia record, any recently completed health history is presented for review within the case, so you can simply confirm and proceed.

  • Flexibility for Past Cases – If reviewing an old case or correcting an error, you can still override health history details for that specific anesthesia record if needed.

Using Another System for Health History?

If you document health history elsewhere, you can continue specifying “Health History Completed Elsewhere,” just as before. Now, however, you also have the option to copy and paste relevant details from another system directly into Xchart. This makes it easier than ever to include necessary health history data from an external EMR into your anesthesia record.
