Sometimes, after a record is finalized, you may realize you want to clarify or add context—without altering the original chart. That’s where Addendums come in.
Unlike our Unfinalize feature, Addendums don’t “undo” or change the original report. Instead, they allow you to add a new page to the finalized PDF—without affecting the original data or audit trail.
✍️ What Addendums Do
Addendums do not change any structured data in the case (e.g. vitals, medications).
They’re for clarifying notes—not corrections.
Addendums appear as a new page added to the end of the finalized PDF.
They are timestamped and permanently attached to the original record.
Only the primary provider on the appointment can create an addendum.
Once an addendum is added, the record can no longer be unfinalized.
✅ When to Use an Addendum
Use an addendum when:
You want to add context or clarification after finalization
(e.g., “Patient called later reporting nausea.”)You’ve already sent the record to insurance and just need to clarify something.
You want to leave a permanent, traceable note without editing the chart.
🛠 When to Use Unfinalize Instead
Use the Unfinalize feature if:
You need to correct structured data like a missed medication or wrong time.
You forgot to enter part of the record before finalizing.
You need to make a change that affects the actual contents of the chart.

📸 How to Add an Addendum
Find the finalized case in the Case Manager where you're listed as the primary provider.
Click the “Addendum” button.
You’ll find it here:
A text entry field will open for you to enter your addendum.
It looks like this:
Once saved, the addendum will be attached as a new page to the original PDF.
If you're ever unsure whether to use an Addendum or Unfinalize, just ask yourself:
“Am I fixing an error, or simply adding a clarification?”
If it’s the latter, an Addendum is the right choice.
