How to Record Level of Consciousness

Xchart supports: Ramsay Sedation Scale (RSS), Glasgow Coma Scale (GCS), and Ellis Sedation Score (ESS)

If you want (or are required to) track a level of consciousness measurement in your anesthesia records, we support the following three common options:

  • Ramsay Sedation Scale (RSS)

  • Glasgow Coma Scale (GCS)

  • Ellis Sedation Score (ESS)


It can be tracked as follows:


Go to the Monitor Setup page:

Click Add Items to Track:

Select the score you'd like to use and click Done (they're toward the bottom of the list):

Now, for each checkpoint in the Vitals you'll be asked to enter a score:

Important note:

Because Xchart will not receive this as a value from the monitor, you'll be asked to enter it for each vitals checkpoint or the checkpoint will be considered incomplete.

So, you can no longer have a fully automatic vitals setup if you choose to chart a sedation level since each checkpoint will require manual entry of a sedation level.
