Within Xchart we have the capabilities for custom consent forms to be added to your organization so that when patient appointments are created you can send the patient the consent forms to have them sign prior to them arriving for their appointment. Currently we only have the ability for a signature at the end, we do NOT support initials or fill in the blanks within the form.
How to create custom consent forms for your organization:
Login to your account at Xchart.com
From the dashboard navigate to "patient forms" on the left side of the screen
Select "create new form"
From this screen you can add your custom consent forms. You would input the information in the text box, shown below. Don't forget you can add your logo!
The below image shows you how the patient would be viewing it from their phone when they receive the link from you.
Note: if you already have PFD or word documents of your consent forms they can be copy and pasted into the text box. You may need to make some adjustments so they can be read correctly in the "preview" section, shown inside the phone, or take out initials and fill in the blanks within the form. But copy and pasting an existing consent form is a quick and easy way to add them to Xchart.
5. Make selections of how you would like this form to be used.6. Select "save form"
NOTE: Custom consent forms can only be created and added to an organization by an admin user. Once this form has been added to the organization then any member can add the said form to a patients appointment.
Additionally, this can be used for post op instructions forms as well. The same process as above, would apply when creating these forms.
