The patient will already need to have an appointment created to be able to add a consent form. The appointment does not need to be "booked" but at least in "proposed" state. Step by step instructions:
Login to your account at Xchart.com
Find the desired patient you would like the sedation consent forms sent to from the calendar on your dashboard.
Open the patient to their "case summary" page.
Select "add consent form"
NOTE: when consent forms are added to your organization by a team member it is an option to always have the form added to any new patient created, if that is the case the forms will already show up under the documents and tasks and you can jump to step 6 to send them.
Select a form you would like to add to the appointment.
Of the consent forms you selected, now back on the case summary page, you would select "send"
Select the best option to send the form to the patient. If you send via their mobile phone simply select "send to" if you have a number on their chart. Or you an input a mobile number or email to send that way.
The consent forms status will go from "created" to "sent" and the patient will get a URL link that will guide them through the process of signing the form.
Once the patient has signed the form you will see it reflected under the case summary page of the specific patient. The status will go from "sent" to "complete."
If the patient has shown up for their surgery but still has not signed their consent forms you can open up their chart to the case summary page, find the form you would like them to sign and select "open." Then they can sign the form on an iPad or touchscreen right then and there.
