How to create a Preset

This article will show you how to do ALL things presets. Creating, editing, applying to a case and applying preset to entire organization.

Presets can be created in a couple of different ways.

  1. After you have gone through an entire sedation case and the provider signs the final document Xchart will automatically ask you if you would like the answers you gave to said case to be saved as a preset. If you select yes, you can then name it and you officially have a preset.

  2. You can create a preset from a "fake case"

Step by step instructions on how to create a preset: NOTE: providers and admins are the only ones that can create/save presets.

  1. Login to your account at Xchart.com

  2. Navigate to the "case presets" tab in the blue bar on the left

  3. Select "build preset using a fake case"

  4. This will then take you into a "fake" chart where you can go through each section and add what you would like to the preset you are working on.

  5. An exhaustive list of all things that can be added to any given preset is below.

The list is as if you are in a patients chart and following along the list in the blue bar on the left.

Under Case Settings:

  1. Sedation Level

  2. Placement of an IV (Yes or No)

  3. Placement of an Airway (Yes or No)

  4. Reason for sedation

  5. What role with the provider be playing during a given case

Under Health History:

  1. Mark all system review answers as "no"

Under Physical Assessment:

  1. ASA classification

  2. Mallampati Class

  3. Chance of Pregnancy

  4. Breastfeeding

  5. Any notes that are typed in typed into the "pre-procedure risk assessment" box

Under Pre-meds:

  1. Whatever premed was given including dosage (you would only then need to click the "press and record time" to timestamp when the premed was taken)

Under Pre-Anesthesia Info:

  1. Last food

  2. Last drink

  3. Recorder Name

  4. Who else is present Name

Checklist: I don't see that you can select "check all" and have it saved as a preset- I am asking you this TC??? It didn't work for me?

Under Monitor Setup:

  1. Vitals types to be tracked- For "vitals to always select" we recommend only adding vital types that your monitor isn't going to send that you may want to track anyway, such as an ECG rhythm or sedation level. It's better to let the "normal" values auto select because that way you can gradually add sensors to the patient, and it will auto-select them as they come in.

Under IV access: You would still need to click the "press and record when IV access is achieved" to timestamp placement of IV

  1. Needle Gauge

  2. Solution Type

  3. Side

  4. Location

  5. Secured with

  6. Attempts

Under Airway Placement:

  1. Any airway you choose to use

Under Patient Position:

  1. Skip or select what position the patient is in

Drugs:

  1. Inhalation agent- can either check box if "starting on room air" or add liters of O2 or N2O if you want that as part of the preset

  2. Parenteral Drugs- select each drug you would like applied to the preset and if you like to have Xchart help you add up your waste you can put in the first draw of each drug as well and that will be saved within the preset.

    NOTE: after you have selected this preset to be applied to a case you would still need to select "record initial draw" under parenteral drugs to timestamp and record data.

  3. Local Anesthetics- select each local anesthetic you would like applied to the preset

  4. Oral Sedatives

  5. Infusion Drugs- select the infusion drug you would like to be a part of the preset and even the units you would like them in.

Anesthesia Info/Notes:

  1. Any notes typed into either free form text box on this page can be saved and applied to a preset.

Recover Info/Notes:

  1. Any notes added to the free form text box in this section can be saved on a preset

Discharging Score:

  1. Discharge score type you would like used

A few tips:

  1. Because you can apply multiple presets at once to any given case, you might choose to save presets into categories. For example, you might save a preset that is *just* for all your normal settings for a naso tube, or... maybe you save a preset with just your regular parenteral drugs.

  2. Presets that are owned by you as an owner are saved to your user account. So if you are part of multiple organizations in Xchart, you'll have your presets on all orgs you are a part of.

  3. Org admins can "change owner" of a preset to share it with the entire organization.

    a. Navigate to "case presets" in the blue bar on the left

    b. find the preset you would like to have applied to the entire organization

    c. select "show details"

    d. select "change owner"

    e. then select "the organization" if you would like every member within the organization to have access to this specific preset.

  4. Non-providers can still save and apply presets to a case. However, they always get saved to the provider account who is listed as the primary on that case. In order to prevent unwanted data loss, we only allow the owner of the preset (the provider or in the case of org presets, an org admin) to edit or delete them.

  5. When you use a preset it simply applies a set of answers as if you had just typed them in yourself. Don't worry, everything is still editable.
